To obtain a form from the Tribunal, please contact the
Tribunal (202.269.9124) directly so that
the proper form(s) for your inquiry can be correctly identified.
If you require any other form that is not shown here,
please submit your request to forms@milarch.org.  
Most of the current forms can be filled in using a Word Processor. Those
requiring signatures and/or attachments must be printed out and mailed to the Archdiocese. Certain
forms can be submitted electronically and are noted accordingly.
PDF Forms
require Acrobat Reader to open. You can fill in these PDF forms
online, and then print and mail them to the usual address.
DOC Forms
(actually DOT templates) require MS-Word to open. After downloading
to your favorite folder, complete the particular document using
your word processor program, print the results and mail it to the
usual address.

RCIA Forms:
These forms are used to obtain important information
about those inquiring about Christian Initiation. A description
of each form follows:
Form RCIA-A:
Inquirer Information Form PDF

This form requests basic contact information, religious history,
marital status, and family information. This is the only form that
inquirers should complete by themselves. The coordinator, a priest-chaplain,
or another initiation minister should complete any other applicable
form(s) in an interview format with the inquirer after he or she
has completed this form.
Form RCIA-B:

If an inquirer is engaged, married, or divorced and not re-married,
specific questions should be asked to determine if marriage-related
issues need to be addressed. Complete the appropriate section of
this form with the inquirer in an interview format.
Form RCIA-C:
Previous Marriage Information PDF

If an inquirer and/or an inquirers fiancée(e) or spouse
has been previously married, use this form to gather basic information
to determine the status of the previous marriage(s). Question (c)
asks specific questions to verify if a Declaration of Nullity has
been granted.
It is best to complete the appropriate section(s)
of this form with the inquirer and/or the inquirers fiancée(e)
or spouse in an interview format, so that you can explain why you
are asking certain questions. Keep in mind that previous marriage
situations, no matter the current status, must be treated with pastoral
sensitivity.
Form RCIA-D:
Christian Initiation Information PDF

The first two sections of this form provide a place to record the
dates and other significant information regarding an inquirers
participation in the Christian initiation process for sacramental
record keeping purposes. Use the reverse side of this form to record
the dates on which the steps of the annulment process have been
completed, if applicable.
Tips for Using the Forms
Photocopy each form on a different color paper to easily distinguish
them.
Keep the forms for those who are currently participating
in the Christian initiation process in a three-ring binder so that
you can insert additional forms as needed.
These forms contain all the information needed
for completing the Archdiocesan sacramental records.
