SACRAMENTAL CERTIFICATE REQUEST NOTIFICATION

The AMS will be performing a Systems Upgrade from 29 July through the second week of August. During this time, the AMS CRM Platform (Constituent Relationship Management) will be shut down for maintenance and we will not be accepting new Sacramental Record Certificate requests.

The Sacramental Records Certificate Request online form will be made available on the website once the new system has been deployed.

In the meantime, please do not call or send fax requests, as the AMS has limited resources. If you have an urgent request that cannot wait, you may submit your request via email to Urgent Request Form – only urgent requests will be processed.

We appreciate your patience.

Unlike a conventional diocese, the AMS has no parishes or parish registries. Because military chapels are United States Government property, all records of sacraments performed on U.S. Military Installations are maintained by the AMS. This starkly contrasts the model used in diocesan parishes where all sacraments are recorded in the parish registry. The AMS currently maintains over 3.2 million records of sacraments performed on military installations.

A staff of 4 full-time employees and 2 college interns perform the task of recording, maintaining and processing requests for the over 3.2 million records.

All requests are required to be made in writing.  Please click one of the links below to request a certificate.

AMS shares the dilemma of many agencies and is challenged by COVID and staffing concerns. Consequently, in a spirit of transparency, you should know that the certificate request turnaround time has increased and will be from 8 to 12 weeks. Every effort is being made to process your requests in a timely manner. Thank you for your understanding.
